There are about 40 kinds of detectable vitamin D metabolites in the body, among which 25-OH-D is the most present metabolite in circulation and the main form of vitamin D (VD) in the circulation, which can reflect the nutritional level of vitamin D in the body. Although the last biologically active form in the human body is 1,25(OH)2D, which is formed through hepatic and renal metabolism, it is not recommended for clinical evaluation due to its rapid metabolism and significantly lower level than 25-OH-D. Therefore, serum 25-OH-D levels are used internationally as an indicator of whether the body is deficient in VD.

25-OH-D plays an important physiological role in osteoporosis, rickets, chronic kidney disease (CKD) and skin diseases, especially in the regulation of calcium and phosphorus metabolism and maintenance of bone health, while its role in muscle, cardiovascular disease, diabetes, cancer, autoimmune and inflammatory responses is also gaining attention.
